Although major surgery may be necessary for some patients, it is not always the first or only treatment option.

What Are Grade 3 Hemorrhoids?

Internal hemorrhoids are commonly classified by how much they prolapse:

Grade 1: Hemorrhoids remain inside the anal canal.

Grade 2: Hemorrhoids prolapse during a bowel movement but return inside on their own.

Grade 3: Hemorrhoids prolapse and usually need to be pushed back inside manually.

Grade 4: Hemorrhoids remain prolapsed and generally cannot be pushed back inside.

Grade 3 hemorrhoids may cause bleeding, pressure, irritation, mucus, swelling, difficulty cleaning, or a noticeable lump after bowel movements.

Do Grade 3 Hemorrhoids Always Require Major Surgery?

Treatment for Grade 3 hemorrhoids without major surgery may be possible for some patients.

Selected patients with Grade 3 hemorrhoids may still be candidates for minimally invasive or clinic-based treatment, particularly when the hemorrhoids are mainly internal and the anatomy is suitable.

However, Grade 3 hemorrhoids are not all the same.

A patient with mainly internal prolapsing hemorrhoids may have very different treatment options from someone with a significant external component, extensive mixed hemorrhoids, large skin tags, or more advanced prolapse.

That is why the first step should be an assessment of the actual hemorrhoid anatomy rather than choosing treatment based on the grade alone.

Non-Surgical and Minimally Invasive Options for Grade 3 Hemorrhoids

Whether Grade 3 hemorrhoids without major surgery can be treated successfully depends on the amount of prolapse, external disease, and individual anatomy..

라파엘로® 고주파 시술

For selected patients, Grade 3 hemorrhoids may be treated without major surgery using minimally invasive options such as Rafaelo® Radiofrequency treatment.

Rafaelo® uses radiofrequency energy to treat suitable internal hemorrhoidal tissue. The treatment is designed to reduce the size and blood supply of the treated hemorrhoid without traditional excisional hemorrhoidectomy.

For appropriately selected patients with Grade 3 hemorrhoids, Rafaelo® may provide a minimally invasive treatment option before major hemorrhoid surgery is considered.

Suitability depends on factors such as:

the location of the hemorrhoids

the amount of prolapse

the presence and extent of external disease

whether the hemorrhoids are primarily internal or mixed

other findings identified during the anorectal assessment

When Might a Minor Surgical Procedure Still Be Necessary?

Non-surgical treatment is not appropriate for every patient with Grade 3 hemorrhoids.

When a significant portion of the hemorrhoidal disease extends below the dentate line, or when there is a substantial external component, a different procedural or surgical approach may be more appropriate.

A more extensive treatment approach may be considered when there is:

a large external hemorrhoid component

extensive combined internal and external hemorrhoids

significant or fixed prolapse

recurrent symptoms despite previous treatment

complications or another anorectal condition requiring surgical management

Some patients with external or mixed hemorrhoidal disease may still have clinic-based treatment options, depending on the individual anatomy and clinical assessment.

The goal should not simply be to “avoid surgery at all costs.”

The goal is to identify the least invasive treatment that is clinically appropriate and likely to be effective for the individual patient.

<p non-life-threatening concerns that require prompt assessment through primary care, while serious or life-threatening symptoms should be assessed through emergency services.

Bowel and Stool Management

Even when a procedure is planned, improving bowel habits remains an important part of hemorrhoid care.

As part of supportive care, our clinic may provide guidance on:

increasing fibre intake

maintaining adequate hydration

reducing straining

managing constipation or diarrhea

avoiding prolonged sitting on the toilet

These measures may reduce ongoing pressure on the hemorrhoidal cushions and may help reduce recurrent symptoms.

Can Grade 3 Hemorrhoids Be Treated Without Major Surgery?

For some patients, yes.

Patients with Grade 3 hemorrhoids may still be candidates for Rafaelo® Radiofrequency or other clinic-based procedures depending on their anatomy, symptoms, and examination findings.

The hemorrhoid grade alone does not determine the most appropriate treatment.

A comprehensive anorectal assessment can help determine:

whether the hemorrhoids are primarily internal, external, or mixed

whether another condition, such as an anal fissure, is contributing to the symptoms

where the hemorrhoidal tissue is located

how much prolapse is present

whether Rafaelo® Radiofrequency may be appropriate

whether another clinic-based procedure may be an option

whether a more extensive surgical procedure should be
